Rather, it emerged through clinical practice, NHS quality improvement work, lived experience, creative collaboration and conversations with health professionals across the UK.</p><p>At first glance, integration can appear to be the final stage of the cycle.</p><p>But integration is not the end of the journey.</p><p>It is the beginning of the next cycle.</p><p>Health is not a fixed state that we achieve once and for all. Learning does not move in a straight line. Both are cyclical processes.</p><p>We notice something.</p><p>We begin to make sense of it.</p><p>We imagine new possibilities.</p><p>We try something different.</p><p>We integrate what works.</p><p>And then life changes, circumstances shift and new challenges emerge. With a new layer of understanding, we begin the cycle again.</p><p>Perhaps this is another meaning of integration.</p><p>Not simply adding another tool, pathway or intervention, but bringing separated parts back into relationship.</p><h3>Integration within ourselves</h3><p>For many people living with persistent physical symptoms, difficulties with emotional regulation or neurodivergent experiences, life can begin to feel fragmented.</p><p>Symptoms can leave us feeling disconnected from ourselves and from those around us.</p><p>Experiences that initially seem unrelated can accumulate over time.</p><p>The story no longer makes sense &#8212; Symptoms and Stress Map</strong><br>Exploring the Symptoms and Stress Map at The Sustainable Healer conference in March 2026. <em>Photo: Jake Gilbert</em></p><p>One of the simplest tools within the Sustainable Practice Toolkit is the Symptoms and Stress Map.</p><p>Rather than asking, <em>&#8220;What is wrong with me?&#8221;</em>, it invites people to begin connecting experiences and symptoms that may previously have felt unrelated.</p><p>When do symptoms flare?</p><p>What was happening in the days, weeks or months beforehand?</p><p>What pressures, responsibilities, relationships, emotions or life events might have been present?</p><p>What patterns begin to emerge when these experiences are seen together?</p><p>The aim is not to suggest symptoms are imagined or &#8220;all in the mind&#8221;. Rather, it is to support people to notice patterns, make sense of their experience and explore the possibility that symptoms may reflect a nervous system that has become stuck on high alert.</p><p>For many people, this process creates a different starting point.</p><p>Symptoms begin to feel less random.</p><p>Experience begins to feel more connected.</p><p>And new possibilities begin to emerge.</p><p>Over time, this can support people to reimagine another way forward and begin integrating new habits, new narratives and new activities into their lives.</p><h3>Integration within the therapeutic relationship</h3><p>Integration can also occur within the consultation itself.</p><p>At its best, healthcare is not simply the delivery of expertise from one person to another. It is a process of making sense together.</p><p>This requires us to bring different forms of knowledge into relationship.</p><p>Professional knowledge and patient wisdom.</p><p>Evidence and lived experience.</p><p>Curiosity and compassion.</p><p>Hope and realism.</p><p>When these perspectives are held together rather than placed in opposition, new possibilities often emerge.</p><p>Not because we have found all the answers, but because we have created enough space to see more of the whole picture.</p><h3>Integration within the wider system</h3><p>Health is shaped by much more than healthcare.</p><p>One of the things I have loved the most about Wild-Ness Health events over the past three years is the breadth of experience in the room. Health professionals, creatives, researchers, people with lived experience and those working in the third sector have all brought a richness to the dialogue that would not have been possible from within healthcare alone.</p><p>This has shaped my own practice too.</p><p>In particular, my connection with the charity <a href="https://www.flipofthecoin.scot/">Flip of the Coin</a> has transformed how I think about what becomes possible when healthcare and community organisations work alongside one another. I first came to know the organisation through my work as an NHS GP and have since become one of its trustees.</p><p>Their creative workshops now take place within view of my consulting window.</p><div places they regularly leave the house for are the doctor&#8217;s surgery, and perhaps the shops on a good day, that can make all the difference.</p><p>This collaboration has taught me about a third form of integration.</p><p>Not simply integrating learning into consultations, but integrating healthcare into the wider web of relationships, places and opportunities that support health and recovery.</p><p>This week I will be exploring some of these ideas at the North Lanarkshire Creative Health Conference in Airdrie alongside Lucy Campbell, Founder of Flip of the Coin and Community Embedded Researcher for <a href="https://gtr.ukri.org/projects?ref=AH%2FZ505456%2F1">REALITIES III</a>, and Dr Marisa de Andrade, Principal Investigator of this UKRI-funded research programme. </p><p>Together we will be reflecting on what becomes possible when healthcare, creativity and community are brought into closer relationship, particularly in communities experiencing significant health inequalities.</p><p>The more I reflect on the Sustainable Practice Cycle, the more I realise that it describes not only how we learn as professionals but also how many people recover, adapt and grow.</p><p>Whether we are navigating persistent physical symptoms, emotional distress, neurodivergent experiences, professional burnout or organisational change, the process often carries a similar rhythm.</p><p>We notice what is happening.</p><p>We begin to make sense of it.</p><p>We imagine what else might be possible.</p><p>We integrate what helps.</p><p>And then, slowly and imperfectly, we begin again.</p><p>Perhaps this is why the Sustainable Practice Cycle seems to resonate across conferences, consultations, coaching conversations, community projects and creative spaces. It does not belong to any one of them. </em></p><p>As May begins, I find myself thinking about creativity.</p><p>As I sit down to write this, I find myself noticing how much creativity has brought into my life over the past few years &#8212; joy, connection, expression, and a way of making sense of experiences that might otherwise have stayed unspoken.</p><p>For most of my life, I don&#8217;t think I would have described myself as especially creative. My training in medicine taught me to look for clarity, structure and explanation. To assess, diagnose and decide. To work with what could be measured, named and acted upon.</p><p>And yet, over time, something has changed.</p><p>Perhaps it began with writing &#8212; trying to find language for things I could feel in practice but couldn&#8217;t easily explain. Or perhaps it came through creating spaces for people to come together and reflect. Either way, I&#8217;ve found myself drawn towards ways of working that feel different from what I was originally trained to do.</p><p>What I hadn&#8217;t expected was how important this would feel.</p><p>There is something about creativity that allows experience to be expressed when sometimes words are not enough. It gives form to things that might otherwise feel stuck &#8212; a feeling, a question, a moment that doesn&#8217;t quite make sense. It doesn&#8217;t necessarily resolve anything, but it makes something visible. And once something is visible, it can be shared.</p><p>I became much more aware of this while preparing for The Sustainable Healer Conference this year.</p><div><hr></div><p>The first afternoon began 2026. </em></p><p>It explored a consultation dynamic that many clinicians will recognise, and many patients will have felt from the other side &#8212; the moment when everything has been ruled out, and yet nothing has been resolved.</p><p>The opening scene was painfully familiar.</p><p>A GP standing outside the door, already carrying a sense of dread.<br>A patient with a long list of symptoms.<br>Tests done. Referrals made. No clear answer.</p><p>As the consultation unfolded, you could feel the recognition in the room. Not exaggerated, not dramatic &#8212; just very real.</p><p>The GP feeling stuck.<br>The patient feeling dismissed.<br>Both of them caught in something that neither quite knew how to shift.</p><p>Later, the audience was invited to ask questions of the two actors on stage about how the interaction had felt.</p><p>At one point, a GP in the audience shared with the patient his frustration &#8212; even anger.</p><p>The impossibility of being expected to make sense of such a complex situation in ten minutes.<br>The sense of being asked to fix something without the time, tools or system around him to make that possible.</p><p>You could feel the shift in the room &#8212; a kind of silent gasp as something that is usually left unspoken was said out loud.</p><p>And then the patient responded.</p><p>With equal frustration, and emotion.</p><p>That this was why she had come.<br>That she needed help.<br>That making this feel okay &#8212; or at least helping her find a way through &#8212; felt like his responsibility.</p><p>That this was his job.</p><p>It was a simple exchange, but it held so much.</p><p>The weight of expectation on the clinician.<br>The vulnerability on the patient&#8217;s side.<br>The gap between what is being asked and what is possible.</p><p>And perhaps most strikingly, the absence of a shared way to navigate that moment.</p><div><hr></div><p>What followed was a reimagining of the consultation.</p><p>Different clinicians stepped into the scene and began to demonstrate small shifts &#8212; not dramatic changes, but subtle ways of being that started even before the consultation began.</p><p>Pausing outside the door.<br>Orientating.<br>Entering the room differently.</p><p>They showed ways of widening the assessment &#8212; moving beyond &#8220;what is the diagnosis?&#8221; to noticing patterns of symptoms, stress and lived experience.</p><p>They experimented with language.<br>With how to offer a working explanation.<br>With how to share power, rather than hold it alone.</p><p>Alongside this, a number of simple consultation tools from the <a href="https://wild-ness.newzenler.com/bookshop">Roadmap to Sustainable Practice</a> were introduced and explored &#8212; an Orientation Card, a Symptom and Stress Map, and a Conversation Guide.</p><p>None of them complicated.<br>All of them small enough to sit within the reality of an NHS appointment.</p><p>And yet, together, they created a noticeable shift in the dynamic.</p><p>Not by solving everything.</p><p>But by changing the direction of travel.</p><div><hr></div><p>What followed this felt like a natural continuation.</p><p>It offered a quieter space to return to what had happened &#8212; not just to ask <em>what do we think about this?</em>, but <em>what did it evoke in us?</em></p><p>Where did we feel it in our bodies?<br>What did it make us want to do?</p><p>Where did we feel ourselves pulled towards doing more &#8212; more tests, more referrals &#8212; even when we knew it might not help?<br>And where did we feel the opposite pull &#8212; towards stepping back, towards &#8220;there&#8217;s nothing more we can do&#8221;?</p><p>Naming those pulls felt important.</p><p>Because they are often what shape the consultation, long before we choose our words.</p><div><hr></div><p>When we returned the next morning, the tone was noticeably different.</p><p>The <em>Healing Landscapes</em> exhibition sat at the start of the day.</p><p>Where the theatre had brought things into sharp focus, the artwork invited people to slow down.</p><p>To wander rather than watch.<br>To pause rather than analyse.</p><p>The pieces themselves had emerged from community creativity sessions in Alness and Invergordon &#8212; created by people whose voices would not usually feature in a space like this.</p><p>And that was the intention.</p><p>To bring lived experience into the room without asking people to expose themselves.<br>Without turning their experiences into something to be consumed or explained.</p><p>Instead, their presence was held through colour, Healer Conference 2026. </em></p><p>People spent time with the work quietly.</p><p>They noticed what stayed with them.<br>They allowed their responses to emerge without needing to immediately interpret or respond.</p><p>It felt like a different kind of listening.</p><div><hr></div><p>Looking back, I think both of these experiences were forms of creativity.</p><p>The theatre allowed us to step into the dynamic &#8212; to feel it, to name it, and to recognise how it can shift when we change the dance.</p><p>The artwork allowed us to step back &#8212; to reflect, to notice, to see more.</p><p>And somewhere between the two, supported by the space to reflect together, something began to shift.</p><div><hr></div><p>As May unfolds, I find myself returning to creativity in a slightly different way.</p><p>Not as something separate from practice, but as part of how we work with complexity.</p><p>A way of finding language when the old words no longer help.<br>A way of staying in the conversation when it would be easier to retreat.<br>A way of opening up possibility, even in familiar and difficult dynamics.</p><p>This month, in the Members Community of Practice, we&#8217;ll be exploring this through the lens of language &#8212; particularly in the context of persistent physical symptoms.</p><p>Looking at how small shifts in wording can change how something lands.</p><p>How they can reduce fear, or create it.<br>How they can either close something down, or open up a different kind of conversation.</p><div class="captioned-image-container"><figure><a Photo by Jake Gilbert.</em></p><p>Looking back now, it feels quite extraordinary to think that this has since grown into a conference that brings together over 60 health professionals &#8212; and people with lived experience &#8212; from across the UK and beyond.</p><p>But what feels most important is not the scale.</p><p>It&#8217;s the relationships.</p><p>From the beginning, this work has been shaped by the people around me.</p><p>Caoimhe, bringing her depth and steadiness through breathwork.<br>Dannii and Jenny, creating spaces of care and nourishment through food.<br>Jake, quietly documenting these moments and helping us see what is unfolding.</p><p>There is something about continuing to work alongside the same people over time.</p><p>A trust that builds.<br>A shared understanding.<br>A sense that we are learning together.</p><div><hr></div><h2>How it grows</h2><p>Looking back, what began in that midwinter gathering was not a fully formed idea.</p><p>It was a set of conditions.</p><p>A space where people could arrive, connect, and begin to think and feel in a different way.</p><p>And from that, something has grown.</p><p>Not in a straight line.<br>Not according to a plan.</p><p>But in cycles.</p><p>A growing awareness.<br>A period of unlearning.<br>Creating space.</p><p>Then something begins to shift &#8212; creativity, reimagining, new possibilities.</p><p>Over time, some of that settles into integration.<br>And at moments, even a sense of flourishing.</p><p>But it doesn&#8217;t always feel like that when you are in it.</p><p>There can be uncertainty.<br>And at times, a surprising amount of fear.</p><p>At the conference this year, we explored how metaphor and imagery can help us make sense of these experiences.</p><p>Jane Gunn describes this beautifully in her book <em>The Mole and the Mountain</em> &#8212; finding your way out of the swamp of murky reality, crossing the bridge of courage, only to find yourself in a dark wood before you can begin to climb and glimpse a wider view.</p><p>That feels much closer to the truth of how change unfolds.</p><div><hr></div><h2>A way of showing up</h2><p>Much of what I have come to understand through this work is reflected in what I think of as the <strong>12 Habits for Sustainable Practice</strong>.</p><p>Not techniques or quick fixes.</p><p>But small ways of showing up.</p><p>Ways of creating safety.<br>Building trust.<br>Making space for sense-making, rather than rushing to resolution.</p><p>They are often quiet. Easy to overlook.</p><p>But over time, they shape what becomes possible &#8212; in consultations, in communities, and in ourselves.</p><div class="captioned-image-container"><figure><a small red berries &#8212; hints of colour returning.</em></p><p>Winter is beginning to loosen its grip here in the Highlands.</p><p>The light is changing. The days are lengthening. Snowdrops are out. There is a quiet sense of anticipation, as we wait for the first signs of spring.</p><p>That shift is rarely sudden.</p><p>In nature, an enormous amount of energy gathers before anything becomes visible. Long before the first leaf appears, something is already underway beneath the surface &#8212; nutrients stored, structures formed, conditions prepared.</p><p>Renewal begins there.</p><div><hr></div><h2><strong>From reimagining to renewal</strong></h2><p>Last month, we explored reimagining.</p><p>The moment we begin to sense that another way might be possible.</p><p>But reimagining alone is not enough.</p><p>Renewal asks something different.</p><p>Not just <em>what might be possible?</em><br>But <em>what conditions are needed for that to take shape?</em></p><p>Because renewal rarely arrives through effort alone. It becomes possible when the conditions are right.</p><p>When we feel supported.<br>When we feel connected.<br>When we begin to feel more like ourselves again.</p><div><hr></div><h2><strong>What winter was quietly doing</strong></h2><p>In the Four Seasons of Learning, winter turns us towards what is happening beneath the surface.</p><p>Not withdrawal, but preparation.</p><p>It invites us to notice three interconnected relationships:</p><p><strong>The inner relationship</strong><br>How we speak to ourselves under pressure. The stories we carry. The moments that help us recognise what is genuinely right for us.</p><p><strong>Relationship with others</strong><br>How we notice the pull towards disconnection or drama, and what helps us stay in relationship when stress is high.</p><p><strong>The therapeutic relationship</strong><br>How we remain present and relational when certainty is limited, especially when our usual tools cannot reliably offer cure or quick resolution.</p><p>Winter is not about stepping back from care.</p><p>It is about continuing to practise &#8212; clinically responsible, and deeply human &#8212; when certainty runs out.</p><div><hr></div><h2><strong>A small act of noticing</strong></h2><p>One of the simplest ways we explored this recently was through a reflection we call the <em>Winter Map</em>.</p><p>Rather than asking, &#8220;How are you coping?&#8221;, it invites a gentler question:</p><p>What sustained you through the winter?</p><p>Who helped you feel welcome and valued?<br>What helped you feel steady and connected?<br>Where did you return to, where your body relaxed and your mind cleared?<br>When did you feel a sense of belonging, or more like yourself?</p><p>Even a few minutes with these questions can begin to shift what we notice.</p><p>Not just what has been difficult &#8212; Last week, around 150 people gathered for a ceilidh. Music, movement, and that unmistakable feeling of belonging.</p><p>A reminder that renewal is rarely individual.</p><p>It happens in relationship.<br>In community.<br>In shared spaces that make something new possible.</p><div><hr></div><h2><strong>Renewal in practice</strong></h2><p>This matters for healthcare too.</p><p>Because sustainable practice is not just about individual resilience. It is about the conditions that allow people &#8212; both patients and professionals &#8212; to feel supported, connected, and able to move forward.</p><p>Many of those conditions sit beyond the walls of clinics and hospitals.</p><p>In community.<br>In creativity.<br>In nature.<br>In relationships that restore a sense of belonging.</p><p>This is why partnerships between healthcare and the wider community matter so much. Not as an optional extra, but as part of what makes renewal possible.</p><div><hr></div><h2><strong>Renewal is not a reset</strong></h2><p>It is not starting again from scratch.</p><p>It is continuing &#8212; with something new emerging.</p><p>Often slowly.<br>Often quietly.<br>Often supported by what has been building for some time.</p><div><hr></div><p>If February was about reimagining what might Photo by Jake Gilbert (@north.westphotography).</p><p>In the Four Seasons of Learning, <strong>reimagining</strong> is our theme for February.</p><p>Not as a promise that everything will be fine, but as the moment we begin to sense that another way might be possible.</p><p>Often, that begins with a shift in attention. Away from everything that feels stuck or not working, and towards what might be possible, even here.</p><h2><strong>When the familiar map no longer fits</strong></h2><p>In everyday care, we begin with a familiar approach.</p><p>We listen for red flags. We look for patterns of disease. We use investigations and treatments to confirm or reverse a specific underlying process.</p><p>This is essential. It is how we recognise serious illness. It is how lives are saved.</p><p>And yet many of the consultations that feel hardest are not hard because anyone has failed. They are hard because the tools we reach for first do not always match the kind of presentation in front of us.</p><h2><strong>Two ways of making sense</strong></h2><p>This is not about two types of people. It is about two different ways of making sense.</p><h3><strong>Disease-based presentations</strong></h3><p>In disease-based presentations, diagnosis often rests on a defining test or structural finding. The clinical decision is relatively clear: you meet criteria, or you do not.</p><p>The diagnosis answers the question, <em>what&#8217;s wrong?</em></p><p>And the language of cure makes sense, because treatment is aimed at reversing a specific process.</p><h3><strong>Pattern-based presentations</strong></h3><p>In many other consultations, there is rarely one single test that provides an answer.</p><p>Instead, understanding emerges from patterns over time: patterns of symptoms, stress responses, context, and impact on daily life.</p><p>Here, the question shifts.</p><p>Not simply <em>what&#8217;s wrong?</em> but <em>what is happening here, what matters, and what might help this person move forward?</em></p><p>This includes many people living with persistent symptoms, functional presentations, differences in emotional regulation, and complex life experiences, with or without co-existing disease.</p><p>This is where our words can either close possibility down, or open something up.</p><h2><strong>When we use the wrong map</strong></h2><p>Problems often arise when we continue to apply a disease-based logic in a context where it cannot deliver what it promises.</p><p>We test. We escalate. We offer treatments.</p><p>Over time, those interventions may become less likely to help, and more likely to cause harm. Not because anyone is careless, but because the map itself keeps directing us towards more action, even when a different kind of response is needed.</p><p>Eventually, we may swing to the opposite extreme:</p><p>&#8220;There is nothing more we can do.&#8221;<br>&#8220;You will have to learn to live with it.&#8221;</p><p>Even when said kindly, this can land as abandonment.</p><p>And so the cycle chronic pain.</p><h2><strong>Language, and what becomes possible</strong></h2><p>At last year&#8217;s inaugural Sustainable Healer conference, one of the most memorable sessions was led by Dr Barbara Phipps: <em>Language Matters</em>.</p><p>Barbara, an NHS GP with a special interest in chronic pain, reminds us of something both simple and profound: what we say in a consultation shapes what happens next.</p><p>Language can amplify fear and reinforce distress. It can also create safety, open understanding, and support change. The impact of those words can endure far beyond the consultation itself.</p><h2><strong>Reimagining what recovery means</strong></h2><p>In disease-based care, cure is often the goal.</p><p>In pattern-based care, a different language is often needed.</p><p>Recovery does not mean the absence of symptoms. It means reclaiming a meaningful, functional life, with or without them.</p><p>I find myself returning to the word <strong>healing</strong>. Not as something vague or mystical, but as a simple idea: the restoration of wholeness.</p><p>Not necessarily the removal of symptoms, but a movement towards connection, meaning, and participation in life again.</p><h2><strong>Why this is not wishful thinking</strong></h2><p>One of the most helpful concepts here is <strong>neuroplasticity</strong>.</p><p>The nervous system&#8217;s lifelong capacity to adapt. To learn. To respond to the conditions around it.</p><p>It helps us understand how patterns of heightened sensitivity can develop over time through stress, adversity, or prolonged strain.</p><p>But it also offers something else.</p><p>If the system can learn patterns that protect us in difficult conditions, it can also learn new patterns when safety becomes possible.</p><p>Not through force. Not through simple positive thinking. But through steady, relational, and contextual change.</p><h2><strong>Reimagining the consultation</strong></h2><p>Many health professionals will recognise the consultation that begins to unravel.</p><p>The long list. The scattered symptoms. The sense that something is not fitting into a single diagnosis.</p><p>Pressure builds. The search continues. Certainty fades. The ways of working that once felt solid no longer quite hold, but a new way has not yet emerged. From the outside, this can look like a loss of direction. From the inside, it can feel disorientating, lonely and hard to know how to move forward.</p><p>And yet this is often where transformation begins.</p><p>It starts with a gentle turning towards what is already present. A willingness to look again at our work, our assumptions, and the stories we carry about ourselves and others.</p><p>Reading <em>Wintering</em> by Katherine May gave language to a season I had already been living through. A recognition that some periods are not about pushing forward, but about standing back. About allowing space for what no longer fits to loosen its grip.</p><p>She writes about emerging from these seasons wearing a different coat. Ideas such as the belief that patients do not want to take responsibility for their health, or that wellbeing is shaped primarily by what happens in the consulting room rather than in the wider context of people&#8217;s lives.</p><p>Letting go of these assumptions did not happen all at once. It unfolded through conversation, through practice, and through paying closer attention to what was already happening in front of me.</p><p>Again and again, I have seen how change becomes possible in relationship. A recognition of beliefs I had not realised were shaping my own identity &#8212; that I was too old to learn something new, or that my contribution was limited to roles that no longer felt quite right.</p><p>As those ideas loosened, something else appeared in their place. A renewed curiosity. A sense of creativity &#8212; not as performance, but as play. Noticing patterns. Connecting ideas. Finding new ways of working within the realities of everyday practice.</p><p>This began to change how I practise as an NHS GP, and how I think about what health and care might look like in the years ahead.</p><p>Wild-Ness Health grew from this process of reflection.</p><p>Not as a strategy, but as a response to lived experience. A way of creating spaces where health professionals could pause, make sense of their work, and explore more sustainable ways of practising together.</p><div
